Dave Stoll worksite located 140 Taylor Mill, Perrysburg, Ohio 43182:  On or about September 17, 2019,   a. The employer did not ensure employees were protected from falls while installing sheathing on two story 10/12 pitch residential structure. The employees were exposed to a fall distance of approximately 13.5 feet from a steep slope roof.   b. The employer did not ensure employees were protected from falls from an unprotected opening on the second floor above the garage. The employees were exposed to a fall distance of approximately 12 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Dave Stoll worksite located at 140 Taylor Mill, Perrysburg, Ohio 43182:  On or about September 17, 2019, the employer did not ensure the side rails of the extension ladders being used on the side of a two story residential structure was extended at least three feet above the landing surface. One employee was exposed to falls while using the ladder to access the roof.
