29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Wagner Metals, LLC, on or about September 17, 2019 at the address of 904 Washington St. Napoleon, OH: The employer did not ensure employees were protected from falls while performing roofing activities for a residential type structure.  Employees were exposed to up to approximately 10 ft. and 20 ft. falls to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   a)      Wagner Metals, LLC, on or about September 17, 2019 at the address of 904 Washington St. Napoleon, OH: The employer did not ensure employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while installing metal sheet roofing on a residential structure. Employees were unable to identify fall hazards on the job site, exposing them to fall hazards of up to 20ft.
