Note: The requirements applicable to construction work under this paragraph are identical to those set forth at '1910.178(l) of this chapter.  29 CFR 1910.178(l)(6): The employer did not certify that each operator had been trained and evaluated as required by paragraph (l).  The certification did not include the name of the operator, the date of training, the date of the evaluation, and the identity of the person(s) performing the training and evaluation.  A) 59 Second Ave Raritan, NJ 08869   The employer did not ensure that the operator of the JLG CE-0148 rough terrain forklift had completed training and had the proper documentation of the training. 29 CFR 1926.701(b): All protruding reinforcing steel, onto and into which employees could fall, was not guarded to eliminate the hazard of impalement:  A) 3963 Rt 1 South Monmouth Junction, NJ 08852  Employees were exposed to impalement hazards from exposed rebar protruding out of the concrete block, that was at the same level of the work platform. The rebar was not capped or protected by any means.   Hazard observed on or about 09/16/19
