29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length, extended over its support more than 12 inches:  Note:  Does not apply if the platform was designed and installed so that the cantilevered portion of the platform was able to support employees and/or materials without tipping or had guardrails that blocked employee access to the end.    a) 301002 Heather Lane Bethany Beach, DE - On or about September 19, 2019, the general contractor failed to ensure that scaffolding used by a subcontractor was properly erected.  Employees of a subcontractor were working on work platforms on a pump jack scaffold which extended more than 12 inches over the support, exposing employees to a fall hazard of approximately 12 feet above grade.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.    a)301002 Heather Lane Bethany Beach, DE - On or about September 19,2020, the general contractor failed to ensure that the guardrails that installed along all open sides of platforms of a pump jack scaffold were properly installed by the sub-contactor.  Where a top rail was installed, a mid-rail and bottom rail were not installed, exposing employees of the sub-contractor to fall hazards approximately 12 feet above grade.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.    a) 301002 Heather Lane, Bethany Beach, DE - On or about September 19, 2019, the employer failed to ensure that the poles of a pump-jack scaffold being utilized by employees of a sub-contractor, were secured to the structure at the bottom.  Employees of the sub-contractor were exposed to fall hazards while working at heights of approximately 12 feet above grade from a pump-jack scaffold on which rigid triangular bracing was not installed at the bottom of the poles.
