29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   a. On September 20, 2019, in the aerial lift, an employee was using a portable grinder to grind concrete tile without safety glasses, exposing himself to being struck in the eye by debris.

29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of American National Standards Institute, B7.1-1970, Safety Code for the Use, Care and Protection of Abrasive Wheels, and paragraph (d) of this section:   a. On September 20, 2019, in the aerial lift, an employee was using a portable grinder that was not equipped with a guard, exposing himself to the hazard of being cut by the blade.
