29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l):  a) Throughout facility: On or about September 23, 2019, and at times prior, the employer exposed employees to struck-by hazards by allowing the Powered Industrial Truck operators to load and unload pallets using a Forklift (Mitsubishi Model FGC25N Serial Number AF82F42504), where the forklift operator training provided had not yet been documented in a certification.

29 CFR 1910.305(g)(1)(iii): Flexible cords used as permitted in paragraph (g)(1)(ii)(C) of 29 CFR 1910.305 were not energized from an approved receptacle outlet:  a) Samples Area: On or about September 23, 2019, and at times prior, the employer exposed employees to electrical shock and fire hazards while utilizing an electrical fan that was connected to an extension cord which was daisy chained with a relocatable power tap and another extension cord before the cords were connected to a branch circuit receptacle.  b) Samples Area: On or about September 23, 2019, and at times prior, the employer exposed employees to electrical shock and fire hazards while utilizing an electrical heater that was connected to a relocatable power tap which was daisy chained with an extension cord before the cords were connected to a branch circuit receptacle.
