29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) Worksite at 26051 Predezzer Lane, Daphne, Al; on or about September 23, 2019: The employer exposed an employee to the hazards associated with falls of 8 feet 9 inches to the ground below while installing metal roofing on a 10:12 pitch roof without using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Worksite at 26051 Predezzer Lane, Daphne, Al; on or about September 23, 2019: The employer exposed an employee to the hazards associated with falls of 8 feet 9 inches to the ground below while getting on/off the roof from an extension ladder which did not extend three feet above the landing area.
