Location: 744 Granite Street Braintree, MA Instance 1: Employees were exposed to a fall hazard of approximately 17-feet, 4-inches from the eave to the ground below, while working on an 9 pitch roof, where fall protection had not been used. Instance 2: Employees were exposed to a fall hazard of approximately 8-feet 4-inches from the second floor to the ground below, while working where window openings were not guarded and no other fall protection had not been used or provided.
