29 CFR 1910.22(d)(1): Walking-working surfaces were not maintained in safe conditions:           3251 Harry S. Truman Blvd., St. Charles: Employer is exposing employees to tripping hazards.  The employees are exposed to unsafe walking surfaces where there are uncovered holes and openings on workplace floors.             On 09/24/2019 employees were exposed to tripping hazards where there were floor openings and floor holes on workplace floors in the Mill area.  One of the floor openings on Diablo Lane was measured at approximately 13 inches by 9 inches.  The depth of the opening was measured at approximately 4 inches.

Truman Bld., St. Charles: Employer is exposing employees to struck-by hazards.  The employees are permitted to operate forklifts which are defective.        On 09/24/2019 employees operated powered industrial trucks which had defects such as, but not limited to, missing slip resistant covers on brake pedals.

29 CFR 1910.179(l)(3)(i): Unsafe condition(s) disclosed during the frequent or periodic inspection(s) required by 29 CFR 1910.179(j) were not corrected before operation of the crane(s) was resumed:     3251 Harry S. Truman Blvd.: Employer is exposing employees to struck-by hazards.  The employees are permitted to operate a crane with defective hook.            On 09/24/2019 a safety latch of the hook of a 500 pound crane was missing exposing employees to struck-by hazard.  The crane was used in Nick's Garage for handling 55 gallon drums of liquids.  The defective crane was not removed from service until the unsafe condition had been corrected.
