The employees are exposed to unsafe walking surfaces where there are uncovered holes and openings on workplace floors.             On 09/24/2019 employees were exposed to tripping hazards where there were floor openings and floor holes on workplace floors in the Mill area.  One of the floor openings on Diablo Lane was measured at approximately 13 inches by 9 inches.  The depth of the opening was measured at approximately 4 inches.

On 09/24/2019 a safety latch of the hook of a 500 pound crane was missing exposing employees to struck-by hazard.  The crane was used in Nick's Garage for handling 55 gallon drums of liquids.  The defective crane was not removed from service until the unsafe condition had been corrected.
