On or about September 23, 2019 three employees installing roof trusses on a 1 story, 7:12 pitch residential home were not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems, exposing employees to 10-foot fall hazard.

On or about September 23, 2019 one employee had placed a six foot A frame portable ladder against the wall in the closed position and was standing on the 4th step to secure truss bottom cords to the top plate of the CMU wall, exposing the employee to a fall hazard.
