29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about September 23, 2019 three employees installing roof trusses on a 1 story, 7:12 pitch residential home were not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems, exposing employees to 10-foot fall hazard.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:  a. On or about September 23, 2019 one employee had placed a six foot A frame portable ladder against the wall in the closed position and was standing on the 4th step to secure truss bottom cords to the top plate of the CMU wall, exposing the employee to a fall hazard.
