(a) On or about September 23, 2019, the employer, Double Vison Services, LLC., did not ensure employees working on the lower first level of the Condo roofs located at 3080, 3086, 3090, and 3096 W. Prentice Ave, Littleton, CO were protected from falling through the skylights located on every kitchen roof of the condo homes. This exposed employees to fall hazards while conducting re-roofing on multiple condos.
