29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. (a) On or about September 23, 2019, the employer, Double Vison Services, LLC., did not ensure employees working on the lower first level of the Condo roofs located at 3080, 3086, 3090, and 3096 W. Prentice Ave, Littleton, CO were protected from falling through the skylights located on every kitchen roof of the condo homes. This exposed employees to fall hazards while conducting re-roofing on multiple condos.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about September 23, 2019, the employer, Double Vision Roofing, LLC., did not ensure employees were protected from falls at a height of 9.1 feet when working on the 1st tier of the kitchen roof adjacent to the skylights, 13.9 feet when working on the first level of the roof, and 18.4 feet to 21.4 feet while working throughout the main roof of the condos while conducting re-roofing construction. The condos are located at 3080, 3086, 3090, 3096 W. Prentice Ave, Littleton, CO.
