On a scaffold at a commercial construction site, located at 207 Miracle Strip Parkway S. in Fort Walton Beach, Florida; On or about September 26, 2019 , the 3rd working level of a tubular frame scaffold system, located on the North side of the building, were not fully planked or decked between the front uprights and the guardrail supports, exposing employees to a 20-foot fall hazard.  b. On a scaffold at a commercial construction site, located at 207 Miracle Strip Parkway S. in Fort Walton Beach, Florida; On or about September 26, 2019 , the 4th working level of a tubular frame scaffold system, located on the East side of the building, were not fully planked or decked between the front uprights and the guardrail supports, exposing employees to a 26-foot fall hazard.

The endpoints at each upright were more than 48 inches (1.3 m) apart.  a. On a scaffold at a commercial construction site, located at 207 Miracle Strip Parkway S. in Fort Walton Beach, Florida; On or about September 26, 2019, the employer exposed an employee to fall hazards greater that 20-foot, in that the cross-braces were used as a toprail, there was no midrail installed, and endpoints at each upright were more than 48 inches apart as the employee performed stucco work.
