Employees used an AHERN Extreme 1045 SN: 1045030690018 to lift materials to the roof of the structure.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  On or about September 27, 2019 at 308 Briarlake CT NE Atlanta, GA 30345:  a) Employer did not develop a safety and health training program to ensure frequent and regular job site inspections were performed in order to eliminate hazards such as, fall hazards, overhead hazards, forklifts, ladders exposing employees to struck-by and falling hazards while performing roofing work.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about September 27, 2019 at 308 Briarlake CT NE Atlanta, GA 30345:   a) Five Employee's engaged in roofing work on the upper level of an apartment complex while standing on the roof edge. The employer did not ensure that the five employees exposed to fall hazards of approximately up to 16' and 7'' above ground level were wearing fall protection.
