Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    a) Tornado Roofing & Gutters LLC at 3833 Wild Elm Way, Fort Collins, CO, 80528: On or about 9/26/2019, each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, a safety net system, or personal fall arrest systems. There were two employees working on a single story roof with a 9/12 pitch with harnesses that were not tied off to anchors and one other employee was not wearing a harness or tied off to an anchor while working on the roof. These conditions exposed employees to fall hazards of approximately 10 ft.
