OSH Act of 1970 Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause serious physical harm to employees: (a) 2975 Galloway Road, Bensalem, PA On or about October 1, 2019, Employer failed to prevent an incident in which an employee took inadequate measures to avoid contact between equipment the employee was testing and a coworker who unexpectedly entered the area in which the equipment was being tested, thereby exposing the coworker to injury from contact with the equipment.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) 2975 Galloway Road, Bensalem, PA, Paver Shop: On or about October 1, 2019, authorized employees performing equipment maintenance, which required the use of energy control devices and procedures, were not trained in all methods and means related to the control of hazardous energies.
