29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) On or about October 1, 2019, employees were exposed to a fall of approximately 14 feet while constructing concrete walls on a new commercial salt storage facility on Vrooman Drive, Painesville, Ohio 44077. The employees were working from a form scaffold and the employer did not ensure that the working levels were fully planked.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use. On or about October 1, 2019, employees were exposed to a fall of approximately 14 feet while constructing concrete walls on a new commercial salt storage facility on Vrooman Drive, Painesville, Ohio 44077. The employees were using a fork lift with a home made scaffold platform to lift employees and materials, the employer did not ensure that it was specifically designed for such use.
