29 CFR 1926.452(j)(7): Where two by fours were spliced to make a pole, mending plates were not installed at all splices to develop the full strength of the member:  Worksite:  The pump jack scaffold was equipped with a wooden pole that was spliced with two by fours without mending plates installed at all splices to develop the full strength of the member.  On or about October 1, 2019, the above pole failed and broke while two (2) were working from the platform of the scaffold, causing the two (2) employee to fall to lower level. One of the employees sustained fatal injuries from the incident.
