29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  a) In the Fab Shop, on or about October 2, 2019 - The employer did not provide a medical evaluation to determine an employee's ability to wear a Bullard Helmet supplied air respirator before the employee was required to wear the supplied air respirator while abrasive blasting inside the blast booth.  Note:  The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator.

29 CFR 1910.134(i)(5)(iv): The employer did not ensure that compressors used to supply breathing air to respirators were constructed and situated so as to have a tag maintained at the compressor containing the most recent change date of in-line air-purifying sorbent beds and filters and the signature of the person authorized by the employer to perform the change:  a) In the Fab Shop, on or about October 2, 2019 - The employer did not ensure that the tag containing the most recent change date of the Bullard Airline Filter, which is in-line of the Bullard Helmet Supplied Air Respirator used in the Blast Booth, was maintained at the Gardner Denver Integra air compressor.  b) In the Fab Shop, on or about October 2, 2019 - The employer did not ensure that the tag containing the most recent change date of the Bullard Airline Filter, which is in-line of the Bullard Helmet Supplied Air Respirator used in the Blast Booth, contain the signature of the person authorized by the employer to perform the change.    c) In the Fab Shop, on or about October 2, 2019 - The employer did not ensure that the Wilkerson Coalescing Filter, which is in-line of the Bullard Helmet Supplied Air Respirator used in the Blast Booth, had a tag maintained at the Gardner Denver Integra air compressor with the most recent change date and the signature of the person authorized by the employer to perform the change.  d) In the Fab Shop, on or about October 2, 2019 - The employer did not ensure that the Bullard Clean Air Box, which contains three filters and is in-line of the Bullard Helmet Supplied Air Respirator used in the Blast Booth, had a tag maintained at the Gardner Denver Integra air compressor with the most recent change date and the signature of the person authorized by the employer to perform the change.
