29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b)  Location: 150 Oakland Street Mansfield, MA  On or about October 3 2019 employees were exposed to fall hazards of more than six (6) feet, where personal fall protection was not used.  Instances:   1. Uncovered floor holes (Primarily on 1st floor - S.W. building but, through out construction) 2. Floor hole covers not secured or marked (1st floor, S.W. building) 3. Unguarded floor openings (elevator shafts) 4. Unguarded wall openings (Main Building - front facing envelope) 5. Missing toe-boards (leading to basement of S.W. building)

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)Note:  29 CFR 1926.1052(c)(7) applies when the top edge of a stairrail system also serves as a handrail.  Location: 150 Oakland Street Mansfield, MA  On or about October 3 2019 employees were exposed to fall hazards where stairways were not equipped with a stairrail system along each unprotected side or edge.
