29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:  a) Workshop, Violation observed on or about December 5, 2019 - Limestone Fabricators, such as stone cutters, were not included in audiometric testing. Employees, who operated a handheld air chisel during limestone veneer manufacturing, were exposed to continuous noise at 280% of the permissible exposure level of an 8-hour time-weighted average (TWA) of 97dBA during the 358-minute sampling period.

Employees, who operated a handheld air chisel during limestone veneer manufacturing, were exposed to continuous noise at 280% of the permissible exposure level of an 8-hour time weighted average of 97-dBA during the 358-minute sampling period.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a)  In the Workplace, on or about October 3, 2019 - The employer did not developed a written hazard communication program where employees were exposed to hazardous chemicals such as limited to: Slipkote 100 Compressor Oil which contained hydrotreated heavy paraffinic distillates, 190 Penetro 90 Spray which contained petroleum distillates, propane and isobutane, and Buff Indiana limestone which contained calcium carbonate and magnesium oxide.
