(a) Southbound 400 to Eastbound 285 Flyover Ramp, Bridge #21, Span #15 at Column #16: On or about 10/03/2019, and at times prior, the employer exposed employees to fall hazards of 54-feet, 4-inches to the lower level, in that the use of the horizontal lifeline system was not ensured at all times while installing permanent metal decking between bridge spans.
