29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):   (a) Southbound I-400 to Eastbound I-285 Flyover Ramp, Bridge #21, Span #15 at Column #16: On or about 10/03/2019, and at times prior, the employer failed to provide frequent and regular inspections of the job site to ensure that the safety program was followed, to include the proper use of horizontal life-line systems and fall protection required on site.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  (a) Southbound 400 to Eastbound 285 Flyover Ramp, Bridge #21, Span #15 at Column #16: On or about 10/03/2019, and at times prior, the employer exposed employees to fall hazards of 54-feet, 4-inches to the lower level, in that the use of the horizontal lifeline system was not ensured at all times while installing permanent metal decking between bridge spans.

29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of 29 CFR 1926.761:  (a) Southbound 400 to Eastbound 285 Flyover Ramp, Bridge #21, Span #15 at Column #16: On or about 10/03/2019, and at times prior, the employer failed to provide fall protection training to all affected bridge workers. The employer could not provide fall protection training certificates, fall protection logs, fall protection training records and or daily, weekly, and/or monthly job site inspections conducted by a qualified person in fall protection.
