29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    The employer is failing to ensure that the eyes and face of employees are protected from flying particles while working on a residential roof.  This most recently occurred on October 3, 2019 on the jobsite located at 230 S. 85th St, Omaha, NE 68114.  Employees were observed using hand-held pneumatic nail guns to install shingles on an existing single-story house without any form of eye or face protection.

29 CFR 1926.307(d)(1): Pulley(s) with part(s) seven feet (2.128 m) or less from the floor or work platform were not guarded in accordance with the requirements specified in paragraphs  (m) and  (o) of this section:Note:  Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than 6 ft. 6 in. (1.976 m) from the floor or platform may be guarded with a disk covering the spokes.     The employer is failing to ensure that all employees are protected from entanglement hazards while working near unguarded machinery.  This most recently occurred on October 3, 2019 on the jobsite located at 230 S. 85th St., Omaha, NE 68114.  The employer has employees using an air compressor that does not have a guard covering the pulleys.

The owner and 4 employees were re-roofing an existing single-story residential house and not utilizing any form of fall protection.  The house has an 8 in 12 steep sloped pitch and ground to eve height is approximately 9 feet.
