29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    The employer failed to ensure that the eyes of employees are protected from flying particles while working on a residential roof. This most recently occurred on or about October 03, 2019 on the jobsite located at 8668 Cedar, Omaha, Nebraska 68124.  Employees were observed using hand-held pneumatic nail guns to perform roofing operations on a residential home without any form of eye protection exposing them to eye injuries.

This was most recently documented at 8668 Cedar, Omaha Nebraska 68124 on October 03, 2019 when employees were observed conducting roofing operations on a residential structure while working at heights greater than 9 feet without fall protection.

This was most recently documented at 8668 Cedar, Omaha, Nebraska 68124 on October 03, 2019 when the employer allowed employees to use a 20 foot aluminum extension ladder that did not extend at least 3 feet past the roof upper landing surface being accessed.

On or about October 03, 2019, at 8668 Cedar, Omaha, NE 68124.  The employer allowed employees to use a 20 foot aluminum extension ladder with the air compressor, removed roofing material and miscellaneous debris too close to the access/egress point of the ladder preventing them from safely accessing and egressing the ladder.
