29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) Roof top: On or about 10/04/2019, and at times prior, the employer exposed employees to eye injuries while using pneumatic nail guns, in that protective eyewear was not provided to an employee operating this tool at 120 PSI while engaged in siding installation activities.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Top of the roof,  Lot Number 11: On or about October 4, 2019, the employer exposed an employee to fall hazards of approximately 15 feet while installing plywood sheathing on the roof of a new single attached family residential construction site without proper fall protection.
