Site - On or about October 12019, employees engaged in roofing activities while working from an elevated ladder jack scaffolding system were not utilizing fall protection and were exposed to falls of over 20 feet.

29 CFR 1926.451(e)(1): scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used:   a. Site - On or about October 1, 2019, employees engaged in roofing activities while working from an elevated ladder jack scaffolding system were not provided with an access ladder where employees were forced to utilize the support ladders as means of accessing and exiting the scaffolding system.
