29 CFR 1910.119(f)(1)(iii)(B): The employer's Standard Operating Procedures did not include precautions necessary to prevent exposure, including engineering controls, administrative controls, and personal protective equipment;    a) At the facility, on or about October 18, 2019 - The employer's written operating procedure HTR +17 Oil Pot, did not address the use of appropriate hand protection in the form of chemical-resistant, impervious gloves to prevent skin contact with ammonia.    b) At the facility, on or about October 18, 2019 - The employer's written operating procedure NH3 Pump P-2, did not address the use of appropriate hand protection in the form of chemical-resistant, impervious gloves to prevent skin contact with ammonia.    c) At the facility, on or about October 18, 2019 - The employer's written operating procedure LTR -25 Oil Pot, did not address the use of appropriate hand protection in the form of chemical-resistant, impervious gloves to prevent skin contact with ammonia.    d) At the facility, on or about October 18, 2019 - The employer's written operating procedure Air Handling Unit AHU #101.1, did not address the use of appropriate hand protection in the form of chemical-resistant, impervious gloves to prevent skin contact with ammonia.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes.  a) In the Machinery Room, on or about September 18, 2019 - The employer did not require employees to use appropriate hand protection while draining oil pots exposing their hands to skin contact with ammonia. Employees wore Ironclad WWX2 gloves while draining oil pots.
