29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  On or about October 4, 2019, at the worksite located at 3208 Federal Highway, Jenson Beach, Florida 34957, the employer did not develop or implement a written hazard communication program for employees who are exposed to hazardous chemicals including, but not limited to, ISO Option 1/2/3, and Redken Color Fusion Permanent Hair Color containing Ammonium Hydroxide are being exposed to upper respiratory irritation, eye and skin irritation.

29 CFR 1910.141(a)(3)(i): All places of employment were not kept clean to the extent that the nature of the work allows.  On or about October 4, 2019, at the worksite located at 3208 Federal Highway, Jenson Beach, Florida 34957, air ducts bringing air into the salon above the hair stylist work area, were not kept free of dirt accumulation, exposing employees to upper respiratory irritation.
