29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded or guards were not designed and constructed to prevent  employee(s) from having any part of their body in the danger zone(s) during operating cycle(s)   On or about September 10, 2019, a temporary employee received a right index finger amputation while operating a Micron America MC-20 Pneumatic Attaching Machine (Grommet Machine).  The employer did not ensure adequate guarding was installed to eliminate amputation hazards at the point of operation.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours:  On or about September 10, 2019, a temporary employee sustained a fingertip amputation to the right index finger while operating a Micron America MC-20 Pneumatic Attaching Machine (Grommet Machine) and the employer did not report the amputation as a result of a work-related injury within twenty-four (24) hours.    There is no abatement certification or documentation required for this item.
