29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) On or about 10/07/19, on a roofing job site, at a house at 508 Norwood Ave., Buffalo, NY: Three employees were doing shingling & flashing work from a ladder jack scaffold erected on the south side of the house. The employees were exposed to a fall of approx. 18 ft. because no fall protection system was being utilized. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.452(k)(5): Scaffold platforms were bridged on to another: a) On or about 10/07/19, on a roofing job site, at a house at 508 Norwood Ave., Buffalo, NY: A ladder jack scaffold consisting of 3 support ladders, one approx. 20 ft. long aluminum pik, one approx. 12 ft. long aluminum pik, and one access ladder was erected on the south side of the house. The shorter pik, which was on the west end of the scaffold overlapped (bridged) the longer pik, which was on the east end of the scaffold by about 2 ft. at the middle support ladder. Three employees were on the scaffolding doing shingling & flashing work. NO ABATEMENT CERTIFICATION REQUIRED
