On or about October 7, 2019, the employer did not ensure that employees were in use of protective helmets while working below the area that other employees were working. This exposed the employees to struck-by hazards.

On or about October 7, 2019, the employer did not ensure that employees were in use of eye protection while using hammers and pneumatic nail guns to install roofing materials. This exposed the employees to struck-by injuries.
