29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end. On or about October 8, 2019, at the jobsite employees were observed working from a scaffold with platform planks that extended over its support more than 12 inches. The employer did not have guardrails installed to prevent employees access to the cantilevered end and this exposed the employees to a fall hazard that was greater than ten(10) feet to the ground below.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. On or about October 8, 2019, at the jobsite, employees were observed working from a scaffold that did not have guardrails installed at the ends. The employer did not ensure that fall protection was provided and this exposed the employees to fall hazards greater than ten(10) feet to the ground below.

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below: On or about October 8, 2019, at the jobsite, employees were observed working from scaffolds that did not have toe boards installed along the edge of the platforms to prevent debris and/or materials from being kicked off of scaffold onto employees working below. Employees were exposed to struck by hazards.
