OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to vessel rupture and struck by hazards:   On or about October 2, 2019, employees were exposed to potential vessel rupture and struck by hazards during the operation of a vessel used to accumulate water and receive pneumatic pressure. The vessel did not have overpressure protection and thereby subject to failure.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).   Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow ASME BPVC.VIIl.1-2019 OVERPRESSURE PROTECTION UG-125 GENERAL requirements.  Another method would be to ensure the water tank is open to atmospheric pressure and not pressurized.
