Tri-County Nursing, LLC, East exit door in Nursing department, on or about October 10th, 2019 and times prior thereto: The east exit door in the nursing department area of the facility was locked with items including, but not limited to, a sliding door lock. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

A written site specific bloodborne pathogen Exposure Control Plan had not been developed. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Tri-County Nursing, LLC, on or about October 10th, 2019 and times prior thereto: Employees had potential exposure to bloodborne pathogens intermittently throughout work operations including, but not limited to, administering tuberculosis vaccination shots and other nursing/personal care tasks with clients. The employer did not maintain a written bloodborne pathogen exposure control plan, therefore, employees had not received training on the employers bloodborne pathogen exposure control plan. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Tri-County Nursing, LLC, on or about October 10th, 2019 and times prior thereto: Employees had potential exposure to bloodborne pathogens intermittently throughout work operations including, but not limited to, administering tuberculosis vaccination shots and other nursing/personal care tasks with clients. The employer did not offer hepatitis B vaccinations at no cost to the employees that had reasonably anticipated exposure to blood or OPIM (other potentially infectious materials). ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).
