29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces were maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.    a) On or about October 9, 2019 and at times prior thereto; Conveyor 1 wooden exterior catwalk platform is not slip-resistant, and is exposed to ice and snow during cold weather months; employees periodically access catwalk to perform maintenance on concrete conveyor system. 29 CFR 1910.25(b)(1): The employer did not ensure that handrails, stair rail systems, and guardrail systems were provided in accordance with 29 CFR 1910.28;  a) Conveyor 1: On or about October 9, 2019 and at times prior thereto; employees access fixed industrial stairs to access a catwalk to perform maintenance and repairs, but a handrail is not installed to prevent employee falls from a 4-5 foot height. 29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:  a) On or about October 8, 2019 and at times prior thereto; employees enter the underground concrete conveyor system house to repair, troubleshoot and maintain two conveyor systems, but employer has not conducted a procedural review since 2002 when the written program was developed.
