29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) On or about October 9, 2019 and at times prior thereto; employees operated a Wood Router CNC: Weeke Optimat BHP008/Vantech 480 which requires blade changes, but written energy control procedures are not in place and equipment does not have periodic inspections.     b) On or about October 9, 2019 and at times prior thereto; employer does not have written energy control procedures for the Butfering SWT-124 wide belt sander.    c) On or about October 9, 2019 and at times prior thereto; employees operated a Park Industries Titan 1000 Granite CNC which requires tool changes, but written energy control procedures are not in place and equipment does not have periodic inspections.     d) On or about October 9, 2019 and at times prior thereto; employees operated a Park Industries Wet Bridge Saw  which requires blade changes, but written energy control procedures are not in place and equipment does not have periodic inspections.         Note* Abatement certification IS required for this item.

29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed.  a) Cabinet Fabrication Area and Granite Area: On or about October 9, 2019 and at times prior thereto; a review of the employer's equipment inventory to include the Weeke Optimat BHP008/Vantech 480, Butfering SWT-124 Wide Belt Sander, Park Industries Wet Bridge Saw and Granite CNC energy control procedures was not completed in the past year, and tooling and blade changes are completed by employees.     Note* Abatement certification IS required for this item.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  a) Cabinet Fabrication Area and Granite Area: On or about October 9, 2019 and at times prior thereto; Employer has not trained employees on energy control procedures. Machines are de-energized at the main power source, but are not locked out for basic maintenance such as cleaning and blade changes; Weeke Optimat BHP008/Vantech 480, and Park Industries Wet Bridge Saw.      Note* Abatement certification IS required for this item.
