29 CFR 1926.451(f)(15)(i): When a ladder was placed against a structure which was not a part of the scaffold, the scaffold was not secured against the sideways thrust exerted by the ladder: a) On or about October 9, 2019, east exterior wall, where a ladder was placed atop fabricated frame scaffolding and against the building without the scaffold being secured against the sideways thrust exerted by the ladder.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: a) On or about October 9, 2019, east exterior wall, where employees working from fabricated frame scaffolding, approximately 15 feet high, were not protected from falling by any type of a fall protection system.
