Foreman Roofing and Construction or about October 15, 2019, at address of 3747 West Alexis Rd., Toledo, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards, such as falls during roofing operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Foreman Roofing and Construction on or about October 15, 2019 at 3747 West Alexis Rd., Toledo, OH: The employer did not ensure that a fall protection training program was in place for employees that are exposed to fall hazards over six feet in commercial roofing. Employees were observed shingling a 4/12 pitched roof without the benefit of fall protection training from Foreman Roofing and Construction.

Foreman Roofing and Construction  on or about October 15, 2019, at 3747 West Alexis Road, Toledo, OH: The employer did not ensure employees were protected from falls while working from a two story commercial structure. Employees were observed shingling a 4/12 pitched low sloped roof without the benefits of fall protection.
