29 CFR 1926.251(a)(2)(ii): Rigging equipment was loaded in excess of its rated capacity:  On October 10, 2019, employees were exposed to a struck by hazard while constructing a high-rise apartment building located at 1600 Euclid Ave., Cleveland, Ohio 44115.  The employees were using a MDT-389 tower crane to lift six steel supports from the 34th floor to the street area below.  The rigging crew used two synthetic nylon straps to lift the beams and the employer did not ensure that the straps were not loaded in excess of their rated capacity.

29 CFR 1926.753(d)(1): Routes for suspended loads were not pre-planned to ensure that no employee is required to work directly below a suspended load except for:  (i)  Employees engaged in the initial connection of the steel; or (ii) Employees necessary for the hooking or unhooking of the load.  On October 10, 2019, employees were exposed to a struck by hazard while constructing a high-rise apartment building located at 1600 Euclid Ave., Cleveland, Ohio 44115.  The employees were using a MDT-389 tower crane to lift six steel supports from the 34th floor to the street area below.  The rigging crew used two synthetic nylon straps to lift the beams and the employer did not ensure that the employee working on the ground was not working directly below the load.
