29 CFR 1926.152(i)(6): In locations where flammable vapors could have been present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition:  (a) Joe's Bakery- 123 North Main Street, Washington, PA: On or about October 13, 2019, employees were using flammable cleaning solutions and a handheld torch to conduct patch work on a roof; the workers ignited a fire from the flammable vapors/liquid and the ignition source of the torch.

29 CFR 1926.24: The employer did not develop and/or maintain an effective fire protection and prevention program at the job site throughout all phases of the construction, repair, alteration or demolition work and/or did not ensure the availability of the fire protection and suppression equipment required by Subpart F of this part:  (a) Joe's Bakery- 123 North Main Street, Washington, PA: On or about October 13, 2019, employees using flammable cleaning solutions to patch a low-slope roof were not equipped with suppression equipment.
