29 CFR 1926.451(b)(4):  Each end of a platform, not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches (15cm):  On 10/4/2019, employees of E.C. Landers Brick Work, Inc. were performing masonry work on a residential building located at Lot 41 Pearl Vista Court, O Fallon, Missouri.  The platform on the fabricated frame scaffold where an employee was working only extended 2 inches over its support.

29 CFR 1926.451(c)(2)(ii):  Unstable objects were used to support scaffold or platform unit:  On 10/4/2019, employees of E.C. Landers Brick Work, Inc. were performing masonry work on a residential building located at Lot 41 Pearl Vista Court, O Fallon, Missouri.  The fabricated frame scaffold used by employees was supported by unstable objects.  Planks of wood and cinder block were used under the scaffold legs.  The employee working on the scaffold platform was approximately 17 feet above the ground level.

29 CFR 1926.451(g)(4)(i):  Guardrail systems were not installed along all open sides and ends of platforms. Guardrail systems were not installed before the scaffold was released for use by employees other than erection/dismantling crews:  On 10/4/2019, employees of E.C. Landers Brick Work, Inc. were performing masonry work on a residential building located at Lot 41 Pearl Vista Court, O Fallon, Missouri.  Employees were working from a fabricated frame scaffold approximately 17 feet above the lower level without installing a guardrail system along all open sides and ends.
