29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect employee health or whenever respirators are required by the employer, a written respiratory protection program that included the provisions in 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented: On or about October 15, 2019 and at times prior thereto, an employee(s) was required to wear a respirator during tasks including but not limited to painting operations, exposing employee(s) to the hazards of respirator use. The employer did not implement or provide a written respiratory protection program including but not limited to the following: a) Ensuring employees were medically cleared to wear a respirator. b) Ensuring employees were fit tested to wear selected respirator. c) Training on proper use and care of respirators.

29 CFR 1910.303(c)(3)(i): Conductors were not spliced or joined with splicing devices identified for the use or by brazing, welding, or soldering with a fusible metal or alloy. Soldered splices were not first spliced or joined to be mechanically and electrically secure without solder and then soldered. All splices and joints and the free ends of conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose: On or about October 15, 2019 and at times prior thereto, two industrial ventilation fans had wiring that was spliced with uninsulated wire nuts, exposing employees to the hazards of electrical shock.
