Shawn Valle Roofing and Construction on or about October 18, 2019, at the address of 144 Township Highway 714, Fremont, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards, such as fall during roofing operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Shawn Valle Roofing and Construction on or about October 18, 2019, was conducting roofing activities at 144 Township Highway 714, Fremont, OH: Four employees were observed tearing off asphalt shingles on a residential structure. The employer did not ensure that a fall protection training program was in place for employees that are exposed to fall hazards over six feet in residential roofing. Employees were observed tearing of shingles on a 8/12 pitched roof without the benefit of fall protection training from Shawn Valle Roofing and Construction.

Shawn Valle Roofing and Construction was conducting tear off operations on or about October 18, 2019, at 144 Township Highway 714, Fremont, OH: Four employees were observed tearing off asphalt shingles on a residential structure. The employer did not ensure employees were protected from falls while working above six feet from the level below.
