29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    a) Jobsite located at 10 Industrial Highway, MS-6 B Complex, Lester, PA 19029: Employees performing servicing and maintenance on machines/equipment are exposed to hazards such as amputations, entanglement and electric shock in that; annual periodic inspection of the energy control procedure was not performed in order evaluate the effectiveness of the hazardous energy control program (lockout/tagout).    Condition was noted on or about 18 October 2019.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Jobsite located at 10 Industrial Highway, MS-6 B Complex, Lester, PA 19029: Clausing drill press - Type MML, Serial#H312502: Employees utilizing a drill press to re-work metal parts are exposed to swarf, entanglement, and amputation hazards without the use of drill shields to guard the rotating chuck and drill bit.    Condition noted on or about 18 October 2019.    b) Jobsite located at 10 Industrial Highway, MS-6 B Complex, Lester, PA 19029: Milford Rivet & Machine Co. - Model#S-423, Serial#725: Employees utilizing a riveter to assemble wheel caster's are exposed to amputation hazards without the use of a machine guard to guard the riveter while in motion.    Condition noted on or about 18 October 2019.    c) Jobsite located at 10 Industrial Highway, MS-6 B Complex, Lester, PA 19029: Taumel Assembly Systems orbital head former - Model#BK-200-NM, Serial#RJAN-2: Employees utilizing a head former to assemble wheel caster's are exposed to entanglement hazards without the use of a front facing machine guard.    Condition noted on or about 18 October 2019.    d) Jobsite located at 10 Industrial Highway, MS-6 B Complex, Lester, PA 19029: Clausing drill press - Model#2274, Serial#2M01408: Employees utilizing a drill press to re-work metal parts are exposed to swarf, entanglement, and amputation hazards without the use of drill shields to guard the rotating chuck and drill bit.    Condition noted on or about 18 October 2019.    e) Jobsite located at 10 Industrial Highway, MS-6 B Complex, Lester, PA 19029: Taumel Assembly Systems orbital head former - Model#BK-200-NM, Serial#XBAM-5-5541: Employees utilizing the automated riveter to assemble wheel casters are exposed to entanglement hazards from inadequate shielding of moving parts. 29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:    a) Jobsite located at 10 Industrial Highway, MS-6 B Complex, Lester, PA 19029: On or about 30 July 2019, an employee was admitted to the hospital as an in-patient for treatment of a finger that was partially amputated while working at a drill press. OSHA was notified of the injury on or about 15 October 2019, more than 24-hours after of the work-related incident occurred.
