29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: USCG Base Kodiak, Hangar 1, C130J Fire Suppression Project: on or about 10 October 2019; employees were observed entering an excavation site with unguarded and/or unprotected vertical walls measuring approximately 12 feet in depth. The excavation walls were not properly sloped for Class "C" soil, and no other form of protective methods exist such as a trench box. This condition exposed employees to hazards associated with cave-ins.
