29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about October 11, 2019, employees were exposed to a struck by hazard while installing roofing materials on a commercial building located at 19495 Bagley, Berea, Ohio 44035.  Employees were working on the roof with tools and materials above other employees on the ground and the employer did not ensure that the ground employees were wearing protective helmets.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about October 11, 2019, employees were exposed to a struck by hazard while installing roofing materials on a commercial building located at 19495 Bagley, Berea, Ohio 44035.  The employees were using air powered nail guns to install the roofing materials and the employer did not ensure that they were wearing protective safety glasses.

On or about October 11, 2019, employees were exposed to a fall of approximately 20 feet while installing roofing materials on a commercial building located at 19495 Bagley, Berea, Ohio 44035.  The employees were using an extension ladder to access the roof and the employer did not ensure that it extended three feet above the landing area.
