29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated  Employees were exposed to fall hazards.  Employees using an aerial lift to access a roof and perform heating and cooling work were exposed to a fall distance of approximately 10 feet from the aerial lift to the ground below.  Employees were not tied off to the boom or basket.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.   Employee(s) were exposed to fall hazards.  Employee(s) engaged in heading and cooling activities were exposed to fall hazards while working at height in the aerial lift man basket.  Employee(s) were not provided with training to recognize, evaluate, and control the hazards, such as fall hazards, associated with working at height in the man basket.  The employees were exposed to a fall distance of approximately 10 feet to the ground below.
