29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a) On or about 10/21/19, on a roofing job site, at a house at 479 West Utica Street Buffalo, NY: One employee was doing flashing work at the chimney on the steep roof on the west side of the house.  The employee was exposed to a fall of approx.  22 ft. because no fall protection system was being utilized. The pitch of the roof was approx. 14 in 12.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about 10/21/19, on a roofing job site, at a house at 479 West Utica Street Buffalo, NY: An extension ladder was set up on the west side of the house and used by employees to access the roof. The siderails of that extension ladder were 30 inches above the roof.  NO ABATEMENT CERTIFICATION REQUIRED
