29 CFR 1926.1051(a):   Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:  Beale AFB, Welcome Center Project: A ladder or other safe means of access for employees entering and exiting a second floor area from a wall opening was not provided.  An employee was observed jumping 4 feet 3 inches from the wall opening to a mobile scaffold platform and was exposed to broken bones.

29 CFR 1926.1053(b)(9): The area around the top and/or bottom of (a) ladder(s) was not kept clear:  Beale AFB, Welcome Center Project: The ground area at the foot of a ladder was not kept clear. On 09/19/2019, an employee jumped approximately 7 feet off an extension ladder to the floor below that was lettered with gravel. The employee slipped after landing on his feet and fell on his back side.
