29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions.  On or about October 23, 2019, employer did not require employees to use appropriate personal protective equipment such as but not limited to respirators with appropriate filter/cartridges to protect them from the inhalation of  substances that contained hazardous chemicals used in the application of TPO (Thermoplastic Polyolefin) membrane on roof.  This condition exposed employees to health hazards from hazardous chemicals.

This condition exposed employees to chemical hazards.

29 CFR 1910.1200(b)(1): The employer did not provide information to their employees about the hazardous chemicals to which they are exposed, by means of a hazard communication program, labels and other forms of warning, safety data sheets, and information and training.   On or about October 23rd  2019, employees who were using hazardous chemicals required to install TPO (Thermoplastic Polyolefin) membrane on a commercial roof were not informed or trained about the health hazards they were exposed from the chemicals they use.  This condition exposed employees to health hazards of hazardous chemicals.
