29 CFR 1910.37(b)(5): Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g. closet):   On or about October 16, 2019, and at times prior, the employer did not ensure that all doors in the shop area and along an exit access were properly identified and could not be mistaken for an exit, exposing employees to entrapment hazards.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  On or about October 16, 2019, and at times prior, the employer failed to ensure all fire extinguishers were identified and readily accessible for use, exposing employees to burn hazards and smoke inhalation.

29 CFR 1910.305(b)(1): Unused openings in boxes, cabinets, or fittings were not effectively closed.     On or about October 16, 2019, and at times prior thereto, the employer failed to ensure that all unused openings on the Challenger Lifts, specifically the control boxes were effectively closed, exposing employees to electrical shock hazards.
